NWMS Achieves 81% Increase in Palliative and Hospice Care Referrals, 68% Increase in Depression Screenings, and 80% Increase in Case Management Evaluations
Background
Northwest Medical Specialties (NWMS), located in the area of Tacoma, Wash. is a 6-site practice dedicated to providing compassionate, high quality and cost-effective medical services to patients with serious infections, malignancies and immune disorders. NWMS’s innovative approach to treatment led them to explore new technologies to improve patient care for their over 1,700 new oncology cases per year.
Application
Through a unique collaboration between Cardinal Health and Jvion, powerful and proven Artificial Intelligence (AI) technology is being made available to oncology practices across the United States. Northwest Medical Specialties (NWMS) is one of the initial sites implementing the solution, driving better care, smarter spending, and improved outcomes.
The initial phase of the project focuses on actioning 30-day mortality insights from the Jvion CORE™. Applied across NWMS’s 6 locations and 1,700 annual oncology cases, the Jvion CORE delivers a prioritized list of patients that includes current 30-day mortality risk, the clinical and non-clinical factors driving the risk, and recommended actions for each individual. The Patient Care Coordinators review the list, focus on patients who require more urgent care, and action the recommendations delivered by the CORE.
Current efforts are driving the Jvion CORE to lower rates of mortality, improve pain management, prevent avoidable deterioration, better identify and treat depression, prevent avoidable readmissions, and lower rates of avoidable inpatient admissions and ED visits. To date, NWMS has improved the use of pain agents, improved palliative and hospice referrals by as much as 81%, driven a 68% increase in depression screenings, and realized an 80% increase in case management evaluations. The Jvion CORE also identified patients at risk of 30-day mortality who would otherwise have been missed by conventional approaches, and led to more timely and meaningful end-of-life care conversations and better utilization of acute care resources.
