Real-World Application of SDOH Community Vulnerability Insights
Top of mind for many health organizations is the use of real-world social determinants of health insights to address health equity and barriers to care. A recent example is an internal analysis of publicly-available census data in the Autumn Wood community of Lincoln, NE. With its analysis, Jvion was able to uncover critical Community Social Vulnerability Insights and surface the non-clinical risk factors negatively affecting community health outcomes in the area. Those risk factors were then tied to recommended actions that can fuel community health action plans, community investments and allocation of scarce resources.
